Registration

To be treated by the Practice you must be a registered NHS patient. We only accept patients who live in our Practice Area. Visitors to the area are accepted as 'temporary residents' for a limited period.

Overseas visitors are treated on an NHS basis for urgent problems only.

Appointments

All surgeries are by appointment only. Telephone the surgery that you normally attend or book online. When booking you will be asked your date of birth so that we can be sure that the appointment is made for the correct person.

Surgery hours are shown in the premises section.

To book online you need to register for Appointments & Prescriptions Online.

Advanced Access

We feel it is important patients can be seen on the same day when they think it is necessary and reserve appointment time to allow this. We can not promise that this will always be with the doctor of your choice or at the most convenient surgery.

If you wish to be seen on the same day please say when making your appointment - alternatively a telephone consultation may be appropriate for you.

We realise that it is also important to allow patients to plan ahead and allow appointments to be booked up to 2 weeks in advance.

Telephone Appointments

We are happy to advise about new or ongoing medical problems by telephone. Many patients find this more convenient than making a surgery appointment.

We offer a telephone consultation as an option when you book an appointment. If this is appropriate for you the receptionist will book a time slot for the doctor to call you back.

Home Visits

If you feel too unwell to come to surgery and think that a doctor may need to visit you at home, please contact Strensall surgery (01904 490532), before 10.00 if possible.

A doctor may telephone you first to discuss your problem and plan the best action.

Please note that we do not routinely visit after a stay in hospital.

Emergencies

Call an emergency ambulance directly by dialing 999 if you have severe chest pain (possible heart attack), serious accident or serious loss of blood.

If the emergency is less serious then phone the Surgery on 01904 490532 or contact the Out of Hours service on 0845 0568060.

Out of Hours

The practice operates from 8 am to 6 pm from Monday to Friday. Emergency medical services outside these hours are provided by North Yorkshire and York Primary Care Trust (NYYPCT).

To get advice or to see a doctor or nurse when we are closed call the Out of Hours service on 0845 0568060 (this number is given on our answerphone). Your details will be taken and a doctor or nurse will call you back to assess the problem. If you need to be seen you will be offered a consultation at the Primary Care Centre based at York Hospital. If you are too ill to attend or are truly housebound you will be visited at home.

Other Health Advice

NHS Direct

If you require medical information or advice you can call NHS Direct on 0845 4647. This is a nurse-led telephone service offering confidential healthcare advice and information 24 hours a day.

If they cannot sort out your problem they will be able suggest the most appropriate course of action.

NHS Walk-in Centre

This is at Monkgate Health Centre, 31 Monkgate, York and is open from 8 am to 6 pm every day. No appointment is necessary. Nurses are available to give treatment and advice about injuries and illness. Telephone 01904 725401 for more information.

Prescriptions

Dispensing

If you live more than one mile from the nearest chemist, we can dispense your medicines without the need to take a prescription to a chemist.

Medicines can be collected from the surgery you normally attend or, by arrangement, from the 'call houses' in Flaxton (Elm Tree Antiques) and Sheriff Hutton (Post Office) during their opening hours.

Repeat Prescriptions

If your doctor has agreed to a repeat prescription this can be requested in writing or online. We do not take prescription requests by telephone. Please allow 2 working days for checking and preparation of prescriptions. A reminder will be printed on the paper slip given with your medicines if you need to make a review appointment.

Please write your request, preferably using the printed sheet attached to the right-hand side of your old prescription. This should be sent, faxed or delivered to the surgery from which you wish to collect the prescription.

Repeat prescriptions can also be requested using Appointments & Prescriptions Online.

Test Results

To get your test results contact the surgery you normally attend one week after your test. Please ring after 2.00pm when staff will be able to give you the information.

For reasons of confidentiality you must telephone yourself as we will not give information about you to anyone else.

Carers

If you look after or support a relative, friend or neighbour then we would like know. Please tell one of our staff so that we can make a note on your record. This will help us to keep in touch with news and information for carers.

Disabled Access

All of our Surgery Premises have been built to conform to Building Regulations. There is provision for disabled parking at all sites. We have access ramps or street level access at all four surgeries.

In addition:

  • We have automatic doors at Strensall and Stamford Bridge surgeries.
  • All surgeries have a door bell at wheel chair height in case patients need to summon help or assistance.
  • To cover distances from the car park, you can arrange for any one accompanying you to borrow a wheel chair by asking at the Reception desk.
We are also mindful of latest guidance and requirements of the Disability Discrimination Act for our patients using all premises.

Complaints

If at any time you feel you have cause for complaint about the treatment you have received from any member of the Practice team then please contact the Practice Manager. We will take all complaints about the service we provide very seriously and we will try to resolve difficulties as soon as possible, informally and in a sensitive way. A copy of the our complaints procedure is available from Reception, if you need more information.

If we are unable to resolve difficulties at the Practice there are other organisations which can offer help:

  • Patient advice and liaison service (PALS)
    If you have any questions, problems or concerns about your care or the care of a person you care for, PALS can advise you on how to make a complaint and act as an advocacy service. Telephone 0800 0688000.

  • Independent Complaints Advocacy Service
    Free service from the Citizens Advice Service and independent of the NHS. Telephone 0845 1203734.

Getting Information

Practice Publication Scheme

As an NHS organisation we have a duty to inform the public about how they can access information about the practice.

Most of the relevant information is included on the web site and printed brochure. Further information is included in our publication scheme document. This is available on our web site or by writing to the Practice Manager.

Patient information

All patient information / medical records are stored confidentially and securely at our four surgery premises. Records are not disclosed to non medical personnel, without written detailed permission. Patients have a right to see their own medical records under the Data Protection Act 1998, but not those of another patient, unless that patient has given written and detailed permission to do so.

To request access to your record please book a telephone appointment with your usual doctor, who will call you and arrange a time for this. Your doctor will need to check your record beforehand. Sometimes not all the information can be provided and, if this happens, an explanation will always be given, e.g. when information is of a sensitive, personal or confidential nature.

If you require a copy of your records there will be an administration charge for this, payable in advance.
